Continuum of Care Permanent Supportive Housing (CoC PSH)
Overview
The CoC Permanent Supportive Housing program provides rental assistance and access to supportive services for individuals, youth, and families experiencing homelessness who have an identified disability verified by medical professionals. This is a long-term solution combining housing with wraparound services.
Eligibility
- Must be experiencing homelessness (or at imminent risk)
- Must have an identified disability verified by a medical professional
- Typically prioritized for chronically homeless individuals
What's Provided
- Rental assistance (tenant typically pays 30% of adjusted income)
- Supportive services including:
- Public benefits acquisition
- Life skills training
- Employment training
- Childcare assistance
- Transportation assistance
- Healthcare linkage
